AGREEMENT AND LIABILITY WAIVER
Participation is possible only if the participant or legal guardian accepts and signs the following waiver of liability, which releases Straight Blast Gym Calgary and its owners, instructors, and agents from all liability both criminal and civil. Applicant affirms that they are in good physical condition and mental health and are aware that all activities, especially those with regard to actual physical contact, are entirely voluntary, and any member can at any time elect not to partake in any activity which they feel may involve some element of risk or discomfort to their person. Accordingly, the applicant accepts all conditions of membership and agrees to hold Michael and Rebecca Sweeney, Straight Blast Gym Calgary, the club’s affiliation, and all other instructors and members harmless from liability for any damages for any injuries, including but not limited to death and disability arising from any activities with Straight Blast Gym Calgary. This also includes risks associated with COVID-19.
Applicant knows there are risks involved in such activities and that unanticipated risks and injuries may arise during such activities. The use of any equipment, facilities, and premises shall constitute acceptance of that risk regardless of the nature of the injury or medical condition.
To maintain a safe environment and health-conscious training, applicant will observe all gym rules and safety guidelines and will use sufficient caution and respect for training partners and themselves during participation. Applicant understands that they can be removed from the gym for failure to comply with Straight Blast Gym Calgary rules which includes, but not limited to, intentionally harming another student and/or unsafe training practices.
Further, Applicant understands that pictures and video may be taken. Applicant agrees and gives permission to Straight Blast Gym Calgary to take pictures and video that may be used in promotions, social media and/or communications.
Applicant further states that they are eighteen (18) years of age or older and are legally competent to sign this release.
I Understand these terms are contractual and not mere recital, and that I have signed this document as my own free act. If a student is under the age of 18 years old a parent/guardian is to sign waiver and agrees to accept all responsibility for the statements above. The terms of this agreement will serve as a release and indemnity agreement for myself and all members of my family including minors while visiting Straight Blast Gym Calgary.
